Mental Health and Counseling Support Referral Forms

The forms below are for individuals requesting emotional, mental, behavioral, and/or academic support.
These electronic forms can be submitted from a computer or mobile device and can be completed in less than 10 minutes. Upon completion, the Mental Health Team which meets twice a month, will review submitted referrals to determine the best interventions needed to address the presenting concerns, and to assign a team member as the point of contact for the referral. Questions regarding the process can be sent directly to Lacey Maddrey, LICSW and Patrice Maites, M.S.
