Class of 2022: Requesting Mid-year Grades Reports
Mid-year grades reports are now available to send to schools. To request that your mid-year grades report be sent to the schools that require it, please fill out the following two forms:

1) Paper form. Available below as a pdf to download, fill out, and print (filename “MidyearGradesRequestForm2.2022.pdf”). Paper copies are also available in the College and Career Center. Please turn the paper form in to Ms. Levenson in the College and Career Center.

2) Electronic form: https://forms.office.com/r/UMA6Cvcc9M

A “Common App Mid-Year Report Form” accompanies every mid-year grade report sent to Common App schools. This form notes any changes that have been made to a student’s Semester 2 schedule since October 1, 2021, such as dropping or adding a course. Students will provide a reason for any changes on their Wilson request forms, and this reason will be provided to colleges.
